Sustainable Wellness: Discovery

Phase one begins with DISCOVERY.  After a thorough assessment to determine your baseline. our therapists will guide you through weekly sessions focusing on discovering YOUR abilities (they may be different than you think,) YOUR goals, YOUR likes and preferences and YOUR barriers and limiting beliefs. 

DISCOVERY
  • Participate in a thorough physical and mindset evaluation.
  • Make healthy goals that don’t create overwhelm.
  • Try new forms of movement that fit your body type and fitness level.
  • Work one-on-one with your therapist to receive physical therapy treatments for any new or pre-existing dysfunctions. 

Sustainable Wellness: Consistency

We’ve all been there.  Starting off strong and doing great and then somewhere along the lines, just losing steam and falling out of the habit… of exercising, of eating right, of doing whatever action we  know will make the change we want. Making a lifestyle change takes effort but you don’t have to do it alone. 

Phase two is all about CONSISTENCY. Consistent action yield results.  Our wellness program offers the community and professional support you need to keep on your wellness path.  Road blocks come up with any journey, whether  it’s an injury, a limiting belief, or a change in life’s circumstances.  We offer the professional support to help you adapt to life’s obstacles and still achieve your wellness goals. 

CONSISTENCY
  • Have support to help you through life’s shifting priorities
  • Be held accountable for showing up and taking action
  • Explore new modes of food intake and energy output
  • Continue with your therapist with any needed PT treatments

Successful lifestyle change is all about navigating the ebb and flow of life’s priorities.  How you come back to healthy choices and actions is what determines your long term success.  

Phase three is all about embodying SUSTAINABILITYIn this phase we focus on providing you with the tools and mindset needed to feel confident in your ability to continue your healthy lifestyle independently.  

SUSTAINABILITY 
  • Less frequent sessions facilitates greater independence and flexibility.
  • Cultivate confidence to maintain what you have learned.
  • Learn tools to continue your lifestyle change— lifelong.

Sustainable Wellness: Pricing

12 Session Package
  • 12 one-on-one training sessions (in-person or virtual) with your Wellness Coach/Physical Therapist
  • 2 Quarterly Workshops. Click here for workshop topics.
  • 3 passes to various community group fitness classes. 
  • 6 months of email support.
  • $1,000 up front or monthly payment of $190/month for 6 months.
  • Add monthly clinic access for scheduled independent workout sessions for $35/month

24 Session Package 
  • 24 one-on-one training sessions (in-person or virtual) with your Wellness Coach/Physical Therapist.
  • 4 Quarterly Workshops. Click here for workshop topics.
  • Option to see other providers for training session.
  • 6 passes to various community group fitness classes.
  • 12 months of email support. 
  • $2,000 up front or monthly payment of $180/month for 12 months.
  • Add monthly clinic access for scheduled independent workout sessions for $25/month.
